Lenbrook Associate Jada Smith Values Collaborating with Team Members to Help Residents with Long-Term Care Needs

Lenbrook provides comprehensive support and care residents may need at every stage of their health journey. Residents value the care they get through the on-campus Medicare-certified Health Care Center that gives them priority access to inpatient rehabilitation services, memory support, and long-term care – and access to caring professionals like Jada Smith and the Long-Term Care team.

Jada is a licensed Practical Nurse with a degree from the Professional Skills Institute in Toledo, Ohio. For more than 21 years, she has helped people with their healthcare needs. She joined Lenbrook three years ago. As the Long-Term Care Floor Manager, she appreciates how the Lenbrook Associates work together to help residents.

“Jada combines her compassion, experience, and leadership skills every day as she guides her team to provide superb care to residents on the long-term care floor,” said John Clark, RN, LNHA and Nursing Home Administrator for Lenbrook. “She collaborates with other healthcare team members and residents’ families to determine the best care plan for each resident. Her caring and inclusive approach extends to her team members whom she encourages to grow in their careers and skills.”

Jada checks in with residents on her floor when she arrives at work as well as with the nurses and certified nursing assistants caring for them. She looks for any needs that have developed and then works to ensure they are successfully met. Often, that means meeting with Lenbrook’s healthcare, social services, and spiritual wellness professionals. They work together to review each resident’s needs and provide resources and information to assist them and their families in navigating transitions.

“I love working at Lenbrook because I get to make a difference for residents in their everyday lives,” Jada said. “Our team works hard to help the residents entrusted to us. Having a smaller patient-to-staff ratio enables us to provide excellent care. That’s a real benefit to being at Lenbrook for residents and associates alike.”
