Rehabilitation Services


Lenbrook’s on-campus Medicare-certified Health Care Center gives residents priority access to inpatient rehabilitation services. Personalized therapy plans are based on individual needs, capabilities, and goals. Residents can continue their progress through post-rehab with outpatient rehabilitation services provided onsite and in the Fitness Center with a personalized exercise program. Whether recovering from an illness, surgery, or seeking enhanced mobility, our 5-Star Medicare-rated Health Care Center provides private, renovated rooms with access to occupational, physical, and speech therapy.

Priority access is given to Lenbrook Residents, and short-term rehabilitation services are also available
to non-residents when space allows.

What’s included


Residents of the Health Care Center are provided the following services:

  • Skilled medical services and 24-hour nursing services
  • Medication and treatment administration
  • Three nutritionally-balanced meals, customized for therapeutic diets, designed by our Registered Dietitian, with snacks available on request
  • Access to licensed physical, occupational, and speech therapists
  • Pharmacy, laboratory, and mobile x-ray services
  • Spiritual Wellness
  • Social Services
  • Regularly scheduled social, fitness, and educational activities
  • All utilities including cable television, local and domestic long distance telephone, Wi-Fi, daily housekeeping with personal laundry and linen service, 24-hour security, and emergency call buttons in the bathroom and bedside
  • Transportation and salon services provided for an additional fee

Admission requirements


For admission to the Health Care Center, a resident must have an examination by a physician and must require rehabilitative services and/or skilled nursing services. Prior to admission, the following documentation is required:

  • A physician’s written order to “Admit to Skilled Nursing Care” and an applicable diagnosis
  • An order for physical, occupational, or speech therapy, if needed (Co-pays may apply based on insurance)
  • For use of Medicare benefit, a three-night inpatient hospital stay is required
  • Medical history and physical, including a statement of current conditions
  • Completed Lenbrook Admission Documents, including financial information and supporting documentation
  • Diet order
  • Medication orders
  • Other documentation may be required at the time of admission
