The best practices care model.

We know that it can be difficult to invite someone into your home or the home of someone you love. We also understand that needs and situations frequently change. You might need around-the-clock assistance or part-time help. Perhaps your health requires ongoing support or a little extra help while recovering from surgery or illness. That’s why we created our client-centered, three-step approach to ensure both quality of care and quality of life:

1. Assessment. A registered nurse with extensive gerontology knowledge always performs our in-home assessments, evaluating health history, physical and cognitive functionality, nutrition, exercise and home safety.

2. Planning. Working with you and (when appropriate) your family and healthcare providers, our nurses design a personalized care plan to select the right Menders by Lenbrook care team for your needs and preferences.

3. Monitoring. RNs review each CNA’s daily narratives. They communicate directly with clients, family members and healthcare providers. And, they revise the care plan when necessary to ensure excellent care and client satisfaction.

“If people need help to recover, this is the company you want. They are unequivocally superb! I had serious surgery and they made the difference in my rate of recovery.”
